Treatment plans and the notes that follow them live in one place in EzPraxis. As mental health treatment plan software, it keeps each plan’s goals, objectives and reviews connected to the progress notes your clinicians write, and it does the same for targeted case management services.

Problem-based treatment plans

  • Build the Initial Treatment Plan from areas of focus, problems, goals, objectives and interventions.
  • Track status and progress over time, with periodic reviews, addendums and discharge.
  • EHR progress notes record progress on each objective. TCM services link to an area of focus and an objective.
  • Bring in the current plans of clients already in care when you start with EzPraxis.

Treatment plan insights

  • Each client’s Treatment Plan tab shows overall progress (the share of objectives met), goals achieved, active objectives, days until the next review, and a timeline.
  • Print a Treatment Plan Summary as a PDF.
  • Dashboard widgets list the treatment plans that are expiring.

Clinical documentation

  • Write intakes, progress notes, assessments, treatment plans, chart notes and other documents. TCM progress notes record each day’s services.
  • Notes move from draft to pending approval, pending client signature, and completed or rejected.
  • Supervisors approve and co-sign. Documents lock, and unlock with the right permission.
  • Every document prints to PDF.

Templates

  • Build your own forms with a drag-and-drop template builder: text, date and time, choices, dropdowns, tables, lists, headers, e-signature and file-upload fields.
  • Add clinical widgets for diagnoses, medications, allergies, psychotherapy notes and treatment plans.
  • Start from the curated template library, and send intake templates to clients through the client portal.

Good to know

  • Treatment plan insights are per client. There are no practice-wide outcome analytics.
  • Documents have no version history. Changes made after a document is completed are recorded in the activity log.
  • The template builder needs a desktop-size screen.
  • TCM progress notes use a fixed form, not a custom template.
  • Bringing in an existing plan is a guided entry for each client, not a file upload.

Frequently asked questions

Can we build our own note templates?

Yes. The drag-and-drop template builder covers fields, tables, e-signatures, file uploads and clinical widgets, and the template library gives you a starting point. TCM progress notes use a fixed form.

Can supervisors approve and co-sign notes?

Yes. Notes can go to pending approval before they are completed, and supervisors approve and co-sign them.

Can we bring our clients’ current treatment plans?

Yes, for clients already in care. Staff recreate the plan’s timeline (the initial plan date, then each review and addendum) and enter its problems, goals, objectives and interventions, so ongoing clients keep their history and their next review date from day one.

Does EzPraxis keep a version history of documents?

No. Edits made after a document is completed are recorded in the activity log instead.
